Welcome the the Summit DD Provider Resource Page. Here you will find resources that we hope will assist you in being a Medicaid Waiver Service provider.

DODD has developed the following tools to assist individuals to navigate through their Provider Certification Wizard (PCW).
Summit DD Provider Sites
- is where the Summit DD posts information on individuals who are looking for providers for the services that they need
- is where all of our active providers are listed. Families, individuals, guardians etc. can use this tool to search for additional information about providers. It is a place where providers can advertise themselves at no expense to them.

Below are examples of Documentation forms that you can use to document services provided to individuals. Please feel free to copy as you like and add your information to them.
The above sample documents may not include all elements required; they are samples that providers may use as templates to create their own documentation system.
Waiver Funding vs Local Funding
- comes from Medicaid dollars and is administered by DODD. As a Certified Waiver Provider for the State of Ohio, you may provide services to any individual in the state of Ohio (in your areas of certification) who has either an IO (Individual Options) or Level 1 Waiver.
- comes from local tax dollars and is administered by the County Board. In order to provide services to an individual in Summit County who is funded with local tax dollars, a provider must have a contract with the Summit DD. Please see the document below for further information on Local Funding and contracts:
Provider Collaboratives
The Summit DD Board facilitates several
Provider Collaborative activities. Most notably collaborative groups for individuals and agencies providing Day Service and Non-Medical Transporation, as well as Independent Homemaker/Personal Care Services. These meetings are held several times a year and allow providers to network with other providers as well as to get information from the Summit DD Board.
The agendas and meeting minutes from these meetings will be posted here on the Summit DD website. All certified waiver providers are invited to attend these meetings.
