Summit DD System User Agreement for:

Tracking System

As a user of Summit DD information management systems or database(s), including Summit DD Tracking System, I understand that I am responsible for: 

·        The security of my user ID (login) and password(s) to any Summit DD computer system for which I am granted access. 

·        I understand that it is my responsibility to protect my password’s confidentiality. 

·        I understand that I must maintain and safeguard the confidentiality of any Summit DD information relating to individuals receiving services that is accessed or obtained in the performance of my authorized duties or activities. 

·        I understand that my access to systems that contain confidential information may be monitored by Summit DD to ensure appropriate access and compliance with system integrity. 

·        I agree to report any suspected security violations, or difficulties with my user ID and/or pass word to the Summit DD Transportation Department promptly.

·        I understand that failure to comply with the above may result in denial of access to Summit DD information systems.
