Share Your Feedback on our 2025-2030 Long Range Plan

Summit DD has gone through an extensive strategic planning process this year. In planning for the future, we sought the input of people and families we serve, our employees, local providers, and other community stakeholders to map out our 2025-2030 Long Range Plan goals. This plan is instrumental in helping us set and achieve the priorities that will ultimately support more than 5,000 people with disabilities and their families, and maintain the trust and confidence our community has placed in us.

Below is a draft of the 2025-2030 Long Range Plan, Vision 2030, (which outlines our long-term goals and priorities) and our 2025 Action Plan (which turns those long-range priorities into action items for 2025). Summit DD values the input of those we serve, their families, providers and the community. Because of that, we are asking for your feedback on our draft of the 2025-2030 Long Range Plan. Please provide any thoughts to help make our plan better using the form below.
