Reporting an MUI
The health and safety of those we serve is vital. If you have a concern about the health or safety of a person with a developmental disability, please contact us though one of the ways below. The more information you can share, the more we can do to put in the safeguards to help protect people.
How to report an MUI:
Employees in the DD Field:
If you are a DD provider (or employed by one) please call 4-hour MUI notifications into the hotline 330-634-8684 (M-F 8:00 am-4:00 pm) or 1-877-271-6733 (afterhours) and submit written reports to MU********@su******.org.
Please remember, as a DD employee you are not permitted to anonymously submit reports of suspected MUIs. However, our Summit DD MUI Department will keep your identity confidential to your employer or other third parties.
Non-DD Employees and Community
If you have a concern about the health and safety of a person you know with a developmental disability, please contact us at 330-634-8684 (M-F 8:00 am-4:00 pm) or 1-877-271-6733 (afterhours). Please note, we do not have emergency dispatch services. If a person is in immediate danger, please call 9-1-1 or your local police department.
When reporting an MUI, Summit DD encourages you to provide your name, best contact information and as many details as possible so we can ensure the best results for the person concerned. Your name and number will be kept confidential and will not be released to anyone outside of the MUI Department at Summit DD. It's important to stress that failure to provide a name and contact number may unintentionally prevent the MUI team from looking into your concerns.
