Person-Centered ISP

Your Individualized Service Plan (ISP)

Your ISP is your plan for the year ahead. It outlines your services, supports, funding, and goals. You and your team help create it based on what matters most to you, and your SSA helps put the plan into action.

Your Role in the Planning Process

You are the most important part of the planning process. We encourage you to take an active role by sharing:

  • What is important to you
  • What goals you want to work toward
  • What supports you need

Your planning team may include family members, service providers, guardians (if needed), or others who support you. Teams meet at least once a year and may meet more often if needed.

Remember, this is your team and your plan. The better we understand you, the better your SSA can help create a plan that supports you.

Tips for successful team planning

  • Stay in Touch: Keep your contact information up to date and respond to your SSA within 10 days. (If your SSA is unable to reach you after two (2) attempts in a thirty (30) day period, or you do not respond to a deadline stated in a letter, Summit DD may conclude that you are no longer interested in receiving services. If your case is closed, you will need to complete the intake process again if/when you decide to receive services from Summit DD in the future.)
  • Keep Appointments: Attend scheduled meetings. Contact your SSA to reschedule or let them know if you’ll be late. Missing appointments repeatedly may lead to a team meeting to review the continuation of your services.

What is an ISP?

An ISP (Individual Service Plan) is a written, person-centered plan that outlines the services, supports, and activities designed specifically for you. It acts as a roadmap to help you work toward your goals.

Your ISP is created with your input, your SSA, and the people who matter most to you, such as family members, providers, guardians (if needed), or therapists. Your team meets at least once a year, and the plan can be updated anytime as your needs or goals change.
