A cross functional team of families, Agency staff and private providers came together to dramatically improve the Individualized Service Plan, or ISP.
The team transformed the ISP document and process (originally a set of documents outlining a person’s services into a larger picture of someone’s life) driven by the individual. The new, more person-centered ISP offered a personal snapshot of the individual’s life with their goals and dreams center-stage throughout. The ISP name, A day in the Life, reflected the more person-centered approach.
