MUI Reporting Reminders

Major Unusual Incidents (MUI):
A Major Unusual Incident (MUI) is any alleged, suspected, or actual incident that adversely affects the health, safety, or welfare of an individual with a disability supported by Summit DD. Summit DD staff take the initial report, ensure immediate actions are in place, investigate the incident, and then ensure that a prevention plan is in place. For alleged incidents of a criminal nature, Summit DD works with local law enforcement. MUIs can be reported to Summit DD by staff, individuals with disabilities, families, providers, or the general public a variety of ways, including the afterhours MUI reporting number.
Provider Compliance:
Provider compliance reviews ensure that providers meet the requirements set out by the Ohio Department of Developmental Disabilities (DODD). Reviews are conducted by DODD and take place at the end of the provider’s term license, or at least once every five years. Reviews can also be initiated or as needed.

Quality Assessment Registered Nurse (QARN):
According to Ohio Revised Code, Summit DD reviews how private providers administer prescribed medications, perform health-related tasks, and complete tube feedings for individuals with specific medical needs. This oversight helps to ensure that best practices are followed during health-related activities and individuals’ safety is top priority. Quality reviews happen at least once every three years, or more often if needed.
Continued Coordination and Monitoring of Services
Service and Support Administrators (SSAs) are responsible for the ongoing coordination and monitoring of an individual’s Individualized Service Plan (ISP). SSAs use person-centered planning to develop, review, and revise an individual’s service plan as well as gauge progress toward desired outcomes (outlined in the ISP). Service plans are reviewed and revised at least every twelve months or more frequently, if necessary.
