Services for Adults
Adults face many decisions in life, such as where to work, where to live, what interests to pursue, or how to make ends meet on a limited budget. Adults with developmental disabilities are no different. Summit DD is here to support you as you navigate the many decisions of life.
Summit DD uses a person-centered planning approach to create your Individual Service Plan, or ISP.
Once you have been found eligible for services, your service and support coordinator (known as SSAs or RSSs) will listen to your goals and help you create a plan to meet them. SSAs are there to help you navigate access long-term supports to set you up for success. SSAs help you understand your options, provide service coordination and ongoing monitoring of services, and help you advocate for what you need.
